Equal parts arrogance and incompetence
The pattern is always the same shape: they tell you to trust them, that it's handled. Then either nothing happens, or what does happen is wrong — and it's on you to catch it, chase it down, and fix it yourself. Not once. Every time. That's not a string of bad luck. That's how the system runs when nobody's checking.
Two tools here. The first helps you write and send a complaint to the right place — about anyone or anything, not just what's on this site. The second isn't about any one company. It's for anyone who thinks the medical system needs to stop asking people to just trust it. One person's account is an anecdote. A thousand people saying "this happened to me too" is a pattern — and patterns are harder to wave off.
If you work in healthcare and want a lever besides individual complaints, there's a directory of real Pennsylvania healthcare unions organizing in home care, hospitals, and nursing homes.
Write a complaint
Works for anything — a company, an insurance provider, a licensed professional, a facility. Pick who it's about and where it should go.
Nothing here is sent to us or stored anywhere — this runs entirely in your browser. It just helps you write a letter and tells you where to send it.

We're patients, caregivers, and family members who are done being told to trust a system that keeps proving it isn't trustworthy — told it's handled, then finding out it wasn't, and left to catch the mistake and fix it ourselves. It doesn't have to be about the same company or even the same kind of problem as anyone else's. If you've lived some version of this, add your name below.
The fix isn't complicated. More double-checking before something goes out instead of trusting the first pass. More people willing to say "I don't know" instead of guessing and hoping it's close enough. And more people willing to say "I was wrong" and actually correct it, instead of going quiet or passing the blame somewhere else. That's the whole ask.
